چکیده

BackgroundSocial Anxiety Disorder (SAD) is the most common anxiety disorder while remains largely untreated. Disturbed amygdala-frontal network functions are central to pathophysiology of SAD, marked by hypoactivity lateral prefrontal cortex (PFC), and hypersensitivity medial PFC amygdala. The objective this study was determine whether modulation dorsolateral activity with a novel intensified stimulation protocol reduces SAD core symptoms, improves treatment-related variables, attention bias threatening stimuli.MethodsIn randomized, sham-controlled, double-blind trial, we assessed efficacy an (20 min, twice-daily sessions 20 min intervals, 5 consecutive days) in two intensities (1 vs 2 mA) compared sham stimulations. 45 patients were randomized three tDCS arms (1-mA, 2-mA, sham). variables (worries, depressive state, emotion regulation, quality life), stimuli (dot-probe paradigm) before right after intervention. symptoms also at 2-month follow-up.ResultsBoth 1-mA 2-mA protocols significantly reduced fear/avoidance worries improved, regulation life intervention group. Improving effect on avoidance state larger than Only threat-related stimuli, symptom follow-up, states, as group.ConclusionsModulation lateral-medial can improve cognitive control, motivation networks might thereby result therapeutic effects. These effects be intensities, though linear relationship between intensity should not concluded. Our results need replication trials.

Fluvoxamine treatment of generalized social anxiety disorder in Japan: a randomized double-blind, placebo-controlled study.

The efficacy of selective serotonin reuptake inhibitors (SSRIs) for the treatment of social anxiety disorder (SAD) has been reported in the USA and Europe. However, no clinical investigation has been done with SSRIs in Japanese patients with SAD. This study was performed to determine the effectiveness and safety of fluvoxamine for generalized SAD (GSAD) in Japanese patients. In this double-blin...

Low Dose Celecoxib Combined with Clozapine for Treatment of Schizophrenia: A Double Blind Randomized Clinical Trial

Introduction: Higher levels of pro-inflammatory cytokines in schizophrenia have suggested that dysfunctions of the immune system may play a role in this disabling condition. The current study was performed to discover if low dose celecoxib combined with clozapine may improve symptoms of schizophrenia or not. Methods:<span...

Vilazodone in patients with generalized anxiety disorder: a double-blind, randomized, placebo-controlled, flexible-dose study

Vilazodone is a selective serotonin reuptake inhibitor and a 5-HT1A receptor partial agonist that is approved for treatment of major depressive disorder in adults in the USA and Mexico. The efficacy, safety, and tolerability of vilazodone for generalized anxiety disorder (GAD) were investigated in a clinical trial (NCT01766401 ClinicalTrials.gov). Participants (18-70 years, inclusive) who met D...

Kava in the treatment of generalized anxiety disorder: a double-blind, randomized, placebo-controlled study.

Kava (Piper methysticum) is a plant-based medicine, which has been previously shown to reduce anxiety. To date, however, no placebo-controlled trial assessing kava in the treatment of generalized anxiety disorder (GAD) has been completed. A total of 75 participants with GAD and no comorbid mood disorder were enrolled in a 6-week double-blind trial of an aqueous extract of kava (120/240 mg of ka...

Esomeprazole for the treatment of erosive esophagitis in children: an international, multicenter, randomized, parallel-group, double-blind (for dose) study

BACKGROUND Acid suppression with a proton pump inhibitor is standard treatment for gastroesophageal reflux disease and erosive esophagitis in adults and increasingly is becoming first-line therapy for children aged 1-17 years. We evaluated endoscopic healing of erosive esophagitis with esomeprazole in young children with gastroesophageal reflux disease and described esophageal histology. METH...
